What should you do if your period is late but your pregnancy test is negative?

A missed period is one of the first signs we usually associate with a possible pregnancy, especially when you’re hoping to conceive. That’s why getting a negative test result can cause confusion or frustration.

However, a missed period does not always mean pregnancy, and a negative result is not necessarily definitive either. To understand what might be happening, it is important to know how the menstrual cycle works and at what point tests can detect the pregnancy hormone.

Why is the test negative if I haven’t had my period?

One of the most common reasons is that ovulation occurs later than usual. When this happens, the entire cycle is shifted and, if fertilisation has taken place, the implantation of the embryo also occurs later.

If the test is taken too early, levels of the hormone hCG (human chorionic gonadotropin) may not yet be high enough to be detected in the urine.

For this reason, when your period is late and the test is negative, it is usually recommended to wait a few days and repeat the test, preferably using your first morning urine.

Factors that can affect the menstrual cycle

The menstrual cycle is a sensitive process that can be influenced by various factors. Therefore, a delay in your period does not always indicate pregnancy.

Among the most common factors that can cause a missed period there are:

  • Stress: increased cortisol can disrupt hormonal balance and delay or even inhibit ovulation.
  • Travel or changes in routine: changes in sleep patterns or lifestyle can affect the regularity of the cycle.
  • Occasional anovulation: in some cycles, the ovary does not release an egg, which can prolong the time until menstruation begins.

Medical causes that can lead to delayed periods

If delays occur frequently and pregnancy tests continue to be negative, it is advisable to investigate possible medical causes that may be affecting the menstrual cycle:

  • Polycystic Ovary Syndrome (PCOS): one of the most common causes of long and irregular cycles.
  • Thyroid disorders: both hypothyroidism and hyperthyroidism can affect hormonal balance.
  • Perimenopause: during this transitional stage towards the menopause, it is common for cycles to become more irregular due to the progressive decline in ovarian reserve.

When is it advisable to consult a specialist?

An occasional missed period is not usually a cause for concern. However, if you are trying to conceive or if missed periods occur frequently, it may be helpful to consult a specialist. It is recommended that you seek medical advice if:

  1. Your period is more than 10–12 days late and a pregnancy test remains negative.
  2. You experience severe abdominal pain or abnormal bleeding.
  3. Irregular cycles become a regular occurrence.

At Barcelona IVF , in the event of a prolonged missed period, we carry out a comprehensive assessment to identify the possible cause and determine the most appropriate course of action in each case, particularly when there is a desire to conceive.
